LONG TERM CARE
Long-Term and Sub-Acute care facilities can expect better patient care, improved accountability and defined return on investment with a complete portfolio of pharmacy-based consulting tools from Clinical Support Services (CSS):
• Enhanced Pharmacy Consulting
• Electronic Medical Records (EMR) for Physician and Nursing Staff
• Prior Authorization Override Service for Medicare Part D and Other Insurers
The federal government requires that all long-term care facilities review patient drug regimens and medical information every 30 days. In most long term care facilities, a pharmacist reviews patient records and creates a recommendation. With the exception of the recommendation, there is little documentation that the pharmacist consultation process has improved patient care or facility viability.
CSS has redesigned the pharmacy consulting process to provide better service and improved accountability. At the core of the Clinical Support Services system is MTMPath™ pharmacy consulting software. MTMPath allows consulting pharmacists to provide traditional consulting services, coordinate prior authorizations and create population-level reports. MTMPath facilitates MDS data collection. In many cases, the data can then download directly into the long-term care facility’s billing software.
Facilities using CSS consulting services have access to EMRPath™. EMRPath is a web-based electronic medical record system. With EMRPath, all facility providers can access patient records online at any computer with internet access.
