Ambulatory Pharmacists

Clinical Support Services (CSS) has designed a proprietary, turnkey solution for providing Medication Therapy Management (MTM) services that utilizes patient data, pharmacy claims (when available) and clinical data to provide a comprehensive pharmacist-based clinical program.

The web-based MTMPath™ clinical documentation software allows pharmacists to efficiently and effectively review a patient’s medications and disease status to assess appropriate therapy and optimize patient outcomes.  Its user-friendly interface guides pharmacists through the data collection, patient assessment and therapy recommendation process so that all care activity can be accomplished in a standardized and evidence-based manner.

Built on a SQL platform, the data stored in MTMPath’s patient registry is easily exportable, queryable and reportable to accommodate the pharmacist’s needs.

Comprehensive Medication Review Using S-O-A-P Note Technology

MTMPath includes a S-O-A-P (subjective-objective-assessment-plan) assessment that streamlines the pharmacist’s evaluation of lengthy drug regimens and clinical components of a patient review. The S-O-A-P assessment walks the pharmacist through the patient’s subjective findings, objective clinical data gathered from the patient and physician, pharmacist assessment and patient plan.

In the S-O-A-P note environment, the pharmacist can quickly generate recommendations that address potential for adverse drug reactions, deviation from standards of care and increase adherence to clinically warranted medications that can improve the beneficiary’s quality of life, decrease symptomatology, and eliminate unnecessary drug expense.

Patient Chart

The Software contains a patient chart for each patient entered into the software. The Patient Chart contains:

DataPath™

DataPath is the Software’s patient registry module.  DataPath allows pharmacists to store and organize patient medical and healthcare records as images and/or discrete data elements.  To the extent possible, registry fields search available databases of drugs, diagnoses, procedures, physicians, etc. in order to minimize text based data input.  The following registry items will be available in the Software to individual pharmacists and independent pharmacies:

MTMPath Standard S-O-A-P Note

The MTMPath Standard S-O-A-P Note for documenting a pharmacist assessment of the patient will be made available in the MTMPath software environment for all users. The electronic S-O-A-P assessment has four distinct areas of evaluation:  Subjective, Objective, Assessment and Plan.

Document Creation

MTMPath provides a standard pdf memo of the S-O-A-P assessment that is made available to all users for correspondence with the physician.  Additionally, the clinician would be able to generate from the assessment a clinical data request to send to the physician.

Document Header

The MTMPath account can be customized to include one (1) text-based account header specific to the account that will be included on the standard S-O-A-P .pdf and clinical data request document generated from the assessment by the pharmacists/users.

Assigned Document Functionality

The Software provides the ability for hard-copy and electronic documents to be uploaded to the Software and attached to the Patient Chart.

Reports

The Software contains a report generator for the ad hoc creation of MTMPath standard reports. Standard reports include the following:

  • Cardiovascular Extract
  • Diabetes Extract
  • Vitals Extract
  • Lab Extract
  • Recommendation Extract
  • Recommendation Summary

Users

Each Client account may have up to five (5) users, including support staff and pharmacy interns.  Each user will have his/her own unique login information into the designated client account. Users are not permitted to share login information.

Additional Packages and Software Features are available should the Licensee wish to upgrade the Software package.
